* These four associations agreed to distribute my questionnaire to their members on my behalf, because their memberships were large (combined over 5000 practitioners) and they cannot provide their membership lists to me for privacy reasons. I received the names of members of all others organizations from the practitioner names posted on the organization websites.

Ethical Clearance
The Conjoint Health Research Ethics Board of the University of Calgary has approved this project. Results will only be released as summaries of the data-in no way will it be possible to identify individual participants.
